This form must accompany your carrier exchange request along with the carrier you wish to exchange and a check, money order, or credit card number to cover the return shipping costs. • Remember to include your name as it appears on the original invoice, mailing address and daytime phone number or email address. All information will be kept confidential and is solely for the use of New Native Inc. Exchanges and Refunds Dept.
  
• Include six dollars ($7.25 + $2.00 for each additional item, in the U.S. only) with a check or money order payable to New Native Inc., or include your credit card number and expiration date. NO CASH PLEASE.
  
• Make sure to let us know what size and color you are exchanging for.
  
• If you request an exchange for an item of higher value, please include the additional cost for the new item you want, along with the $7.25 exchange fee.
